Parents and guardians have two choices for beginning the enrollment process:

  • Gather the necessary documents below and visit Lake Region High School  to obtain a paper enrollment packet and follow the “Enrollment Procedures” below,
    –OR-
  • Gather the necessary documents listed in the “Enrollment Procedures”  below and use the Parent Portal to initiate the registration process electronically, then visit Lake Region High School to complete the process

If you wish to use the electronic process through the Parent Portal to save some time in the registration process, please follow these instructions:

  • Visit the Parent Portal
  • Log in using your existing Parent Portal account –or- create an account if you do not already have one.
  • Click the menu item titled “Preregistration,” then follow the on-screen prompts.
  • If you need assistance with the use of Parent Portal to preregister your student, please contact the Polk County Schools IST Help Desk at 863-519-8040 or Help Desk

Additionally, the Parent Portal  is geared specifically towards parents and offers access to Online Grades, Attendance, Bus Delays and Required Immunizations, Lunch Account, Communication with teachers and much more.

Enrollment Procedures

If enrolling for the first time in Polk County Public schools, you will need:

  1. Birth certificate
  2. School physical (less than 1 year old unless coming from another Florida school district. Then, you may use a previous physical which may be requested from the previous Florida school.
  3. Current updated immunizations and the immunization records on the Florida Certificate of Immunization Form 680. The Polk County Health Department, Free Clinics, and Private Physicians have access to the Florida Certificate of Immunization. They will transfer immunization record.
  4. Two current proofs of residence in the enrolling parent or legal guardian’s name. A Parent will need to present custody paperwork for a student if appropriate and/or requested. A legal guardian is required to show District approved legal documentation of guardianship. You must present two different proofs of residence from two separate categories below.
    • Florida’s Voter ID or Registration (must be within one year and/or not expired).
    • A current (within the last 90 days),  utility bill (cable, electric, phone, gas – for home use)
    • Lease agreement, rent receipt (with name, address, date, contact information, and signatures on it), mortgage, deed, closing paperwork, FEMA, housing authority, or Joe Tedder, Tax Collector, Property Taxes. Property Taxes must be the most recent.
    • Proof of government benefits such as social security, disability, children and family services, child support letter (dated within the last 90 days).
    • Florida Driver’s License, Proof of automobile registration, or Automobile Insurance bill/statement. Insurance must be within the last 90 days).


If any TWO of the documents listed above cannot be provided because the parent and student live with someone else, an Affidavit of Residence may be used. The Affidavit of Residence must be notarized by a notary public of the State of Florida. Two proofs of residence from the owner/lease holder who signs the form will be needed.

The Request for Temporary Assignment form must be completed to accompany the notarized Affidavit of Residence, and both forms submitted to the zone school along with the required two proofs of residency for processing. The Request for Temporary Assignment and Affidavit of Residence must be approved by the Pupil Accounting Department before the student will be allowed to start school.

5.  Student must be zoned for Lake Region High School or have a District approved transfer on file.

       6. Completion of Lake Region High’s School Registration Packet.
If returning to Polk County Public Schools, you will need:

It is strongly recommended to bring all appropriate transfer paperwork, transcripts, IEP, 504 Plan, withdrawal paperwork, andW contact information from the previous school so not to delay the enrollment and/or registration process.
 

If your student is “zoned” for LRHS then we will be able to enroll and register when the previous documentation has been provided.  Different situations require different information, so please read each category to determine which is appropriate. Please be patient as we be certain all paperwork is in order. After the student is enrolled and activated, the assigned counselor will review documentation and schedule the student.
